General

Adult Disability Medical Healthcare Individuals living with developmental disabilities experience significant health and healthcare disparities. ADMH was founded to address these disparities and provide a comprehensive interdisciplinary team approach to addressing healthcare and social service needs in this patient population. ADMH partners with Urban Family Practice Associates (UFPA), utilizing the Patient Centered Medical Home model. All patients begin their healthcare experience within our ADMH team. From there, options are available to receive primary care within a medical home model at Urban Family Practice Associates or to serve as a comprehensive consultative service to patients who have a primary care physician. ADMH is one of a few clinics nationwide offering care for individuals with developmental disabilities in a community based setting and offering both primary and specialty care.

GA IPSEC is a group of people dedicated to ensuring that every Georgia student has access to learning after high school, regardless of intellectual or developmental disability.

North Atlanta Speech Therapy We are a disability advocacy company providing speech pathology, puberty/sex education, & relationship coaching based out of Atlanta, Georgia.

The Arc’s Center for Future Planning Toolkit offers free resources to help self-advocates, families, professionals, and others tackle all aspects of future planning, including housing, finances, employment, support services, and more.

Day Programs

Annandale Village  dedicated solely to to providing progressive life assistance to adults with developmental disabilities.  Located in Suwanee.

Creative Enterprises we provides day programs along with social and life skills training to individuals with disabilities. This is accomplished through on and off-site work training, job placement assistance, educational programs and community involvement events; along with a variety of additional resources.  Located in Gwinnett, Dawson, White & Forsyth Counties.

InCommunity  more than 650 employees who provide all levels of support in the community to people with a variety of needs; including residential support, employment services, respite care, family support, in-home care, and day programs. To date, InCommunity has provided support and service to more than 10,000 Georgian community members.  Located in Atlanta, Marietta, Norcross, Conyers, & Roswell.

easterseals Adult day service centers provide a place outside the home for older adults and younger adults with all types of disabilities to be active in the community, socialize with their peers and receive needed health and personal care services.

Exceptional Foundation of Atlanta we foster lifelong learning, emphasizing active, hands-on, age-appropriate learning with real life applications.

Great Prospects, Inc. provides post-secondary (after high school) and summer programs for adolescents and young adults with developmental disabilities. Great Prospects, Inc. helps adolescents continue to grow and thrive, and young adults to make the critical transition to the adult world while learning to become a contributing member of the community. Located in Roswell.

Harmony House Day Program is a part-time mobile program for young adults 18+ with intellectual disabilities. We provide pick-up and drop-off services to all participants.  Our focus is to create a social interaction by exploring the Metro Atlanta area and offer peace of mind for family members.

Hi-Hope provides quality services that empower adults with intellectual and developmental disabilities to define and live a meaningful life.  Located in Lawrenceville.

Friends U Need, affectionately called “Fun Club” by its members, offers a safe and structured daily enrichment program Monday – Friday, 9:00 AM – 3:00 PM. We provide recreation, community service, personal enrichment and daily life skills education activities as well as transportation to, and from, our facility.  Located in Buford.

Just People, Inc  a Support Program for Adults with Disabilities

Karen Heart Foundation Established an Adult Day Care Center in response to the needs of disabled adults and their caregivers.  Located in Conyers.

Limitless Disability Services We believe every individual deserves independence and inclusion in a safe and loving environment. The day program gives special needs adults the ability to be independent and grow as an individual through community support and activities. Located in Woodstock

Next Step Ministries  fills the gap in services for special-needs young adults who have completed high school and are not able to participate in a job coaching situation, or other day program. They also offer Saturday respite care, school break care and summer camps.  Located in Woodstock.

Ohana Adult Day Program We are a licensed community-based setting providing adult day health care services to seniors and disabled adults who may require supervision during the day and/or assistance carrying out daily activities.  Located in Loganville.

Sevita we believe that everyone is capable of reaching new heights and thriving in the community. Our services for adults with intellectual and developmental disabilities (I/DD) focus on personal choice while recognizing the specialized, and often complex, needs of the individuals we serve.  Locations throughout GA.

Stacey’s Place, Inc. Helping adults with intellectual & developmental disabilities and their families by providing respite care, residential living and a day program so they can grow towards more independent and fulfilling lives. Located in Cumming.

The Exceptional Foundation of Atlanta  provides purposeful activities for adults with disabilities to increase social interactions, community service, and healthy lifestyles.  Serving an inclusive community for individuals with a wide variety of both physical and mental disabilities such as Down syndrome, Autism Spectrum Disorder, Developmental Delay, Cerebral Palsy, etc.  Located in Snellville.

United Day Program provide support to adults with intellectual disabilities and ensure that all individuals advance towards their independence despite their disability. We educate and advocate for the rights of each individual. Our services are based on the individual’s needs and desires and is unique to each person’s desired goals. United Day Program strongly believes that together with family, caregivers and a good support team, each individual can dream their own dreams.  Located in Conyers.

United Cerebral Palsy  Everyone enjoys spending time exploring their community, making friends and learning new things. UCP’s Adult Day Services meet this desire by offering life enrichment opportunities. Supports emanate from “hub” locations that operate on weekdays in the following communities: Conyers, Atlanta, Macon, Augusta, Glennville and Savannah.  Individuals may select from a variety of enriching pursuits which include exploring leisure activities such as art and music, enhancing community living skills, building skills in preparation for a career in the work world, and volunteering with local organizations. The core focus of daytime activities is to support a person  in discovering their gifts and talents and helping them match those with opportunities to enrich their lives.

YALLS group is a 501(c)(3) non profit day program, guiding adults with mild to moderate disabilities in the workplace, community, and home.

Residential

Camps

Camp Infinity-FOCUS (Age 14-29; Rutledge, GA) Each summer, FOCUS offers Camp Infinity so teens and young adults with disabilities can enjoy a week of camp at Camp Twin Lakes Rutledge.  Campers gain independence, enjoy friendships with peers, and try new outdoor adventures.  Campers must complete a weekend of FOCUS Camp Octoberfest to attend.

Camp Big Heart-Session 1 is an overnight summer camp for adults with mild to moderate special needs, sponsored by the Civitan Clubs of Georgia.  Located in Fort Valley.

Camp Blue Skies serves adults aged 21 and older with cognitive and developmental disabilities.  Located in Rutledge.

Camp Dream  Camp Dream offers recreational Summer Camp and Camp Out programs for children & adults with moderate to severe physical and developmental disabilities. Camp Dream programs are 100% volunteer, providing a 1:1 camper to counselor ratio during its Summer Camp sessions. Camp Dream offers a barrier free environment and has never turned away a camper due to the severity of their disability or their financial capability.

Camp Sparrowwood Where All Abilities Are Celebrated.  Sparrowwood is a chance for campers with different developmental needs to be accepted for who they are and celebrated for all they have to offer. Focusing on abilities rather than disabilities fosters the shared sense of radical acceptance that helps define the Glisson experience – causing all involved to see we are more alike than we are different. Young Adults age 18-39 & Adult age 40-60.  Located in Dahlonega.

Exceptional Foundation of Atlanta Summer Program Join us for sports and rec, music therapy, arts and crafts, and a variety of special interest groups, but most of all enjoy being a part of a community of friends! The Exceptional Summer Program is for ages 16 and up.  Sign up for individual weeks or all three.   9am to 2pm Monday through Thursday.  Summer dates only.  Located in Snelville.

Lindsey’s Place Camp (Age 7-30; Savannah, GA) offers a safe and fun place for your camper to make memories and new friends. We have two on-site nurses available 24/7 during camp sessions and provide 1:1 camper to counselor ratios when needed, however, there is one counselor for every two campers at all times. You can rest assured your camper is safe and taken care of while you enjoy a much deserved respite from continuous caregiving.

OctoberFest – FOCUS (Age 14-29; Winder, GA) Each fall, FOCUS offers OctoberFest, a one-night retreat for teens and young adults.  Participants enjoy fun, outdoor activities and renew friendships.

Supported Employment

Bobby Dodd Institute We offer a variety of training and certification programs to help you succeed in any industry and job placement services for work-ready job candidates.  Includes family support.

Briggs & Associates matches the abilities of people with disabilities to real jobs, then provides the support and training needed to ensure success for both the employee and employer. We provide comprehensive employment services for any individual who may require non-traditional support and/or entry into the workplace.

Frazer Center Frazer Center’s Community Access Program provides supports designed to help each individual explore their unique place in the community. Our staff provides guidance and support that allows each participant to work towards maximizing their potential to become productive, contributing members of the community.

Java Joy offers teens and young adults to part-time jobs in a supportive environment. Java Joy is a nonprofit founded by Extra Special People (ESP) that aims to employ individuals with disabilities.

Just People, Inc  a Support Program for Adults with Disabilities

Peachtree Farm is to produce locally grown produce, products and animals for the metro Atlanta and North Georgia area while providing job opportunities for individuals with disabilities.

PRISM Where special people thrive. Our transition programs help individuals with special needs build long-term independence and become contributing members of the community.  Located in Roswell.

Roosevelt at Warm Springs to empower individuals with disabilities to achieve personal independence and employment success.

Tommy Nobis Center helps individuals with all types of disabilities to enter or return to employment, and to enjoy productive and independent lifestyles while contributing to the greater business community.  Located in Marietta.

The Shine Community a place for people with all abilities to work, play, learn, belong and experience the hope and love of Jesus together.  We offer physical & recreational activities, team sports, hands-on job training and opportunities to serve and build up one another.  Located in Duluth.

Vocational Rehabilitation (VR) provides services to help eligible persons with disabilities prepare for, start and maintain competitive employment, thus becoming productive and independent citizens in their communities.

WOW powers futures by providing life skills training, community integration, and employment training and support. Choice and Advocacy are at the forefront of all program models. “We Work Together as a Team!” and in so doing, we become stronger ourselves and with our community.  Located in Tucker.

Respite

Life Behavior Consulting  We believe everyone has it already within them to work toward their goals! LIFE Behavior helps teens and adults learn the skills to take action steps toward goals and overcome challenges through training, coaching and support.  We provide virtual services individually or within group classes!

Peter & Paul’s Place provide opportunities for adults with intellectual disabilities to gather together to live, socialize and learn in a supportive and inclusive community.

The Next Stop Foundation provides opportunities for our members to socialize, enjoy recreation, and continue to learn three afternoons a week.  Located in Suwanee.
