Equipment Grant Program

FOCUS provides grants for children with developmental and physical disabilities who would benefit from the equipment to support participation in daily life. 

Eligibility
  • FOCUS Kid must be between ages 0-21 AND
  • Have a physical and/or developmental disability that requires medically necessary equipment.
  • Are a Georgia Resident.
  • The total lifetime per child is $10,000 (Loaned equipment does not count towards lifetime maximum or grant per year limit) 
  • Van-lift grants are one-time only grants, with a maximum of $10,000 (Please see guidelines below for those applying for van lifts)
  • Ceiling lift and stair lift grants are one-time only grants per child
  • Families can apply for one grant a year per child
  • Equipment must be purchased by FOCUS. We cannot pay for equipment already owned by the family 
  • Family income must be below 350% of the federal poverty level. See chart below for more information.
FOCUS provides grants for the following types of equipment:
  • Van lifts, ramps, behind the vehicle lift systems** 
  • Transport style wheelchairs as a manual backup (ex. Convaid). If requesting as a primary wheelchair, you should pursue the wheelchair through your insurance funding (private insurance or Medicaid). 
  • Activity chairs (ex. Tumbleform, Special Tomato, Rifton). *Include an insurance or Medicaid denial letter to be considered. 
  • Materials for ramps for home access (Installation not funded)
  • Stair lifts 
  • Patient care lifts *Include an insurance or Medicaid denial letter to be considered. 
  • Custom bath equipment  *Not covered by insurance or Medicaid 

**The FOCUS Equipment Committee has established the following guidelines to consider before completing a grant application for a wheelchair accessible van:

  • If a child currently uses a motorized wheelchair (power chair), they are eligible to apply regardless of weight
  • A child weighing less than 50 lbs is not eligible for a grant with FOCUS unless there are extenuating circumstances
    • Therapists can describe extenuating circumstances in the letter of medical necessity
    • Families can describe unique family situations in their personal letter with the application
    • Families are encouraged to reach out to the FOCUS Equipment Coordinator ([email protected]) to discuss extenuating circumstances before completing the full application
FOCUS does not provide grants for the following types of equipment: 
  • Adapted car seats  
  • Therapy 
  • Exercise equipment (Ex. adapted bicycles, adapted tricycles, treadmills, therapy balls)
  • Hospital beds, Pediatric safety beds 
  • Medical equipment (Ex. feeding pumps, suction machines, oxygen concentrators, pulse oximeters, specialty formula, etc.)
  • Computers, Augmentative communication systems 
  • Hearing aids
  • Glasses 
  • Home modifications (Ex. construction) 
  • Standers
  • Gait trainers, Walkers

  • Approval Process: Applications are reviewed by our Review Committee. Applications must be complete to be eligible for review. If a family’s application is approved, it may take up to 9 months for funding to be released. Once funding is released, FOCUS will pay the vendor directly. Families are never paid directly. 
  • Process: To begin the Equipment Grant process, you will need to complete the Equipment Grant Request form. Once you have completed the request form a FOCUS staff member will contact you. If you are eligible, you will then receive our Grant Application. Along with filling out the application, you will need to provide a Letter of Medical Necessity from either a therapist or doctor, a photo of your child in their current wheelchair (if applicable), A letter written by you about why your child needs this equipment, and either a Current Tax Return, SSI letter, or SNAP eligibility (first two pages, SSN’s removed). All of this is outlined in the Application.
  • Completed Applications: Scanned and emailed to [email protected] or mailed to: Devi Knapp ℅ FOCUS, 3825 Presidential Parkway, Suite 103, Atlanta, GA 30340. 
  • Van Lift or Conversion Grants: FOCUS may cover up to $10,000 for van lifts and conversions.   To add a ramp to an existing van (that families already own), the mileage must be less than 90,000 miles.  To purchase a van, the family must qualify financially; this determination is usually made by the van company.  FOCUS will cover up to $10,000 towards the lift; checks are always made to the van company 
  • Stair Lifts, Ceiling Tracks, Home Ramps and Other Home Modifications: You must own your home to be eligible for these types of grants.
  • Deadlines for Completed Applications:
    • January 15th
    • March 15th
    • May 15th
    • July 15th
    • September 15th
    • October 15th
