The purpose of this workshop is to help families prepare a petition to file for the Appointment of a Guardian of an Adult. This workshop is not for those who are trying to decide if they need/want to do a guardianship, this workshop is for those who have determined that they need or want to do a guardianship for their adult child. The adult child must be diagnosed with a developmental disability (this is not for mental health guardianships). Seating is limited so those in attendance will be able to get their petitions in order and be prepared to file. In preparation for this workshop, please bring: Most recent Psych-Ed eval, Developmental Eval, or equivalent. Address and phone number of the proposed ward (adult disabled child) Address and phone number of spouse and adult children of the proposed ward (if any) Address and phone number of parents (both biological parents must be identified), if divorced and both parents are not in agreement, this workshop is not for you, you will likely need an attorney to assist you. Address and phone number of at least 2 adults who are familiar with the needs of the proposed ward and would agree that the proposed ward needs a guardian and that the proposed guardians are the best people to be the guardian.
